Please do not share detailed medical information or test results via this form; your case will be evaluated during the consultation.

Before and after your appointment

  • Please bring all of the patient's previous examinations to every appointment, in printed or electronic form (CD, USB drive, etc.).
  • Please bring the patient's medical file and all previous reports.
  • Please bring all medications used up to the appointment date, or a list of them.
  • If you are unable to attend or will be late, please let us know in advance.
  • During the consultation, ask again about anything you did not understand or are unsure of, and take notes if necessary.
  • When leaving the hospital after treatment, make sure to obtain your discharge summary (epicrisis), treatment reports, test results, pathology results if any, and the treatment plan printout after Gamma Knife or CyberKnife treatment, and bring them to every follow-up visit.
